Get it laminated at an office supply store. Fold in half, or quarters (you can use a business card document template and fit it on there). Put with driver's license in wallet. Update as necessary. Finally, while the hospital might be 20 minutes away, an ambulance is almost always closer - and brings the ER to you! Never, ever take the chance with chest pain! Please! One thing I learned was to keep a pair of sweat pants and a sweat shirt in there. I refused to wear the awful hospital gowns (you know, the kind that are supposed to tie in the back but always come untied in the middle of a hallway in front of everyone?) so I wore ratty t-shirts and yoga pants.
